Day Break Cares is a program of Catholic Charities of Santa Clara County, a 501(c)(3) public charity. Established as a means to both create jobs for unemployed and underemployed individuals and increase income for under-resourced charitable services for seniors, Day Break Cares is a fee based enterprise that provides non-medical home care, adult day care, and care coordination services to seniors and disabled adults.
Day Break Cares opened doors in 2002 to meet a growing demand for care for older adults with Alzheimer’s and dementia who needed supervised care while providing respite for family caregivers. Our first Senior Activity Center was established in partnership with the Saint Francis of Assisi Parish in San Jose where it remains. Later in 2008, we opened our second senior activity center in Sunnyvale in partnership with First United Methodist Church in Sunnyvale. In a continued effort to be responsive to the needs of families, in 2010 we expanded our menu of services to include in-home care services.
Our goal is to help seniors stay in their own homes, to improve and enrich vital, independent living, to promote dignity and self-respect, and to encourage participation in community life. Our services promote 1) staying independent, 2) staying healthy, 3) staying connected and 4) advocacy for senior needs and concerns. To date, we have supported over 900 seniors to stay healthy and extend their independence in their homes for at least 6 months, and avoid or delay institutionalization.
As an employment focused enterprise, Day Break Cares partners with Foothill College to train and employ unemployed and under-employed individuals seeking to become Geriatric Health Care professionals. To date, we have trained 250 geriatric home aids, and employed 79 Geriatric Home Aides.
About Catholic Charities of Santa Clara County: A non-profit organization, serving over 40,000 individuals annually through 40 different direct service programs. Established in 1955, CCSCC’s mission is to serve and advocate for families and individuals in need. Through 80 different community sites, CCSCC provides a wide variety of services such as: older adult services, mental health services, immigration legal assistance, refugee resettlement, supportive housing, job skill training and placement, ESL and computer training.
Through the Older Adult Services Division, CCSCC serves over 15,000 seniors annually along a continuum of care, which emphasizes independence, health, and advocacy for seniors. CCSCC develops and implements innovative services adapted to the specific needs of older adults and their families. Programs include: home care, adult day care, senior wellness centers, food programs, long term care ombudsman, behavioral health, senior nutrition programs, and handicapables program, etc.
